The CHR disclosed to the public that the following conditions to exist in and about the above noted food establishment which are or may become injurious or dangerous to the public health or which might hinder the prevention or suppression of disease, namely:
a. Owner/Manager does not have approved food safety training,
b. Mouse droppings present on floor in back area of kitchen below desk,
c. Accumulation of dust/dirt present on and around ceiling air vents,
d. Floor is very dirty throughout kitchen, specifically in hard to reach areas and near back exit door,
e. Area where chemicals are stored behind dishwasher is dirty,
f. Shelves throughout kitchen are very dirty,
g. Inside of cupboard and drawer located below hand wash sink in kitchen are very dirty,
h. Overall condition of kitchen is extremely unsanitary,
i. No written cleaning and sanitizing procedures or schedule in place,
j. Surfacing on wooden counter unit has come off,
k. Various items stored on kitchen shelves not associated with food preparation or operation. Area is unorganized and cluttered preventing effective cleaning,
