This contravenes the AR 31/2006 Section 3(2) which states: Subject to section 2(2) and Parts 3 and 5, no person shall handle food except in an approved food establishment.

  1. The facility located at unit 1B 506 10 Street NW is not identified as an approved facility under F.A.T.S Bar & Grill Food Handling Permit issued by the Calgary Health Region. This contravenes the AR 31/2006 Section 3(1) which states: No person shall operate a food establishment unless a person is an operator who holds a valid and subsisting permit for the operation of the food establishment, and the food establishment is identified on the permit as approved by the regional health authority that issued the permit.
  1. A door has been installed between unit 2 506 10 Street N.W (F.A.T.S Bar & Grill) and unit 1B 506 10 Street NW providing direct access between the two facilities without the notification or approval of the Calgary Health Region. This contravenes the AR 31/2006 Section 6 (b) which states: A permit holder must notify the regional health authority that issued the permit in writing regarding any change in the operation of or major renovations or alterations to the food establishment in respect of which the permit was issued.
  1. Many boxes, bags and containers of food are being stored directly on the floor in various areas throughout facility. This contravenes the AR 31/2006 Section 23(1)(a)(b) which states: All food used or to be used in a commercial food establishment must be protected from contamination, and handled in a sanitary manner.
  1. Open pizza dough is being stored unprotected on back shelves which contravenes the AR 31/2006 Section 23(1)(a)(b) which states: All food used or to be used in a commercial food establishment must be protected form contamination, and handled in a sanitary manner.
  1. Walk-in cooler light does not work which contravenes the AR 31/2006 Section 17(1)(f)(iii) which states: An operator must ensure that, before operation of the commercial food establishment commences, the commercial food establishment is equipped with lighting that is adequate in intensity to enable to sanitary operation and maintenance of the food handling areas,
  1. Ceiling tiles throughout facility are removed or missing which contravenes the AR 31/2006 Section 17 (1)(a) which states: An operator must ensure that, before operation of the commercial food establishment commences, the commercial food establishment is of sound condition and in a good state of repair,
  1. Heavy accumulation of grime and debris is on floors, walls, all counters, shelves, equipment surface, walk-in coolers and in hard to reach areas which contravenes the AR 31/2006 Section 17 (1)(a) which states: An operator must ensure that, before operation of the commercial food establishment commences, the commercial food establishment is of sound condition and in a good state of repair.
