The CHR disclosed to the public that the following conditions to exist in and about the above noted food establishment which are or may become injurious or dangerous to the public health or which might hinder the prevention or suppression of disease, namely:

a. Equipment, shelving, floors, food storage bins and walls in the facility are extremely dirty with food debris, grease, and/or rust build-up.

b. Ready-to-eat foods are stored directly beside raw meats in the walk-in cooler.

c. No written records of perishable food temperatures, sanitizer strengths, and cleaning schedules are being kept.

d. This facility does not have any means to measure the sanitizer strength of the chlorine dishwasher or the counter-top sanitizer.

e. There was no sanitizer measured in the cleaning buckets or the sanitizer spray bottle.

f. There was no sanitizer measured in the chlorine dishwasher.

g. An owner or staff member who has care and control of the facility does not have approved food safety training.

h. Food preparation counters are damaged and lifting and many shelves have dirty cardboard and newspapers lining them.

i. Foods were being left out at room temperature.

j. Dishwasher was leaking large amounts of water onto the floor.

k. Stacking food containers without a physical barrier between food items.

l. Rice stored in a dirty rusted garbage can.

m. Pest control measures are not being documented.

n. After handling raw meat, staff did not wash hands before performing other food related tasks.

o. There is no written sanitation program in place.
