The CHR disclosed that the following conditions to exist in and about the above noted food establishment which are or may become injurious or dangerous to the public health or which might hinder the prevention or suppression of disease, namely:


a. Collection of water sample(s) from the water holding tank and submission of

sample(s) laboratory analysis has not been carried out.


b. A garden hose from a neighboring business was being used to fill the water

holding tank.


c. The potable water system and hot water tank were both inactivated, therefore, there

was no readily available means for food handlers to wash hands or clean and sanitize equipment.


d. There was no provision in place for emptying the wastewater holding tank.


e. There was a build up of grease on the fume hood, fume hood filters, grill and deep fryer.


f. The Operator was operating without health approval and without a valid health permit.
